Contact CRA via personal income tax enquiries at 1-800-959-8281. If you did receive a slip, they may have it on file so you can obtain the slip information to include on your tax return.

If they do not have the slip on file at this point, and you do recieve it after filing your taxes, you can always do a T1 adjustment. You do not want to fail to report income as CRA will charge you a penalty.

Call, if not continue your taxes and then file it in when you receive it later. you have up to 5 years to claim whatever it is you didnt claim last 4 years ago.

go to revenue canada for more info

the form to add more doc later on is in "forms' in the canada revenue website

first of all, why you think you should receive a T5?

Anyway, you can call your financial institution where you think you T5 slips are from.
